Sacramental Record Request Form

Sacramental records are released only to individuals who received the sacrament, to requesting church institutions, or to parents of minor children in question. Records created 85 years prior to January 1 of the current year are available for genealogical and other research. If the record you are requesting is not found in the Archives, you will be notified of other possible locations or referred to other repositories as soon as possible. The suggested fee for research and transcription is $15.00 per record.
